Nurses work with physicians to provide home care, assessments, skilled treatment and support to provide the high quality of care you need to recover at home. A Registered Nurse is available for admissions, urgent situations or any questions 24 hours a day. Wound Vac, Wound care, Diabetes education and management including insulin administration, IV therapy, Colostomy and ileostomy care, Catheters care and changes, Venipunctures/ lab draws, Feeding tube care, Medication teaching and management. Our nurses provide teaching for patients and families regarding what to expect with their disease process. They teach patients and caregivers signs and symptoms of exacerbation of their disease state, so that they can catch symptoms early and manage them. This is imperative, in keeping our patients out of the hospital.

A home safety assessment is conducted on admission visit and recommendations are made to promote safety in the home. This reduces falls and injuries.

This care plan is used for patients who are diagnosed in the physician's office and sent home
• Skilled Nurse will provide daily visits for 5-7 days
• Monitor vital signs, lung sounds and O2 saturation.
• Provide patient and/or caregiver education regarding disease process.
• Provide patient and/or caregiver with education regarding medications. Provide medication reconciliation and management.
• Education regarding nebulizer.
• Education regarding what to avoid (Smoking, aerosols, etc.)
• Home Health Aide to see patient, as needed.
• Offer therapy as needed to assist patient with strength and endurance.
• Provide physician updates communication note every other day for seven days.
This care plan is used for patients who are diagnosed in the hospital or facility and have been sent home.
• Skilled Nurse will provide daily visits for 5-7 days.
• Monitor vital signs, lung sounds and O2 saturation.
• Provide patient and/or caregiver education regarding disease process.
• Provide patient and/or caregiver with education regarding medications Provide medication reconciliation and management.
• Education regarding nebulizer.
• Education regarding what to avoid (Smoking, aerosols, etc.)
• Focus on regaining baseline clinical status, prior to hospitalization.
• Home Health Aide to see patient, as needed.
• Begin therapy for strengthening and endurance as soon as medically advisable.
• Provide physician updates communication note weekly for three weeks.
